The Disability Access Center is transitioning to a new database system called Accessible Information Management (AIM). In order to have a profile in the new system, we need all students to fill out an application in AIM. We have added the questions for the Fall 2019 Authorization to Distribute Access Plans and Emergency Evacuation Request Form to the end of the application to reduce the number of forms that you will need to fill out for the new system! You are not starting over with the DAC! This is just an initial step to get you added into AIM!
If you have already completed an application in AIM or completed an Authorization Form for Fall 2019 PRIOR to August 10th your user profile has been created in AIM. Please check with DAC staff to check if your Access Plan has been sent.
The purpose of the Authorization to Distribute Access Plans and Emergency Evacuation Request Form is to notify instructors of a student’s accommodations, it is necessary to present the instructors with a student’s Access Plan. As the Disability Access Center (DAC) generally will not release protected information without a student’s explicit permission, students are required to complete this form to indicate which instructors they wish to receive the letters every semester.
