1. Student will schedule the exam/quiz with Disability Access Center (DAC). 2. DAC will notify the instructor of the exam date and time scheduled and request the exam/quiz. 3. Instructor will complete this form, attach form and exam/quiz, and deliver to DAC – email is fine.
