If you will be utilizing your accommodations in the upcoming Maymester or Summer semesters, please make sure to fill out the Authorization to Distribute Access Plans Form and Emergency Evacuation Form. Completing this form tells the Disability Access Center which instructors you want to receive your accommodation letters for the semester and whether you will need assistance in an emergency situation. When filling out the form be sure to list all instructors for the course (if a course has more than one instructor please list all of them), include instructors from any lab sections, and provide complete information and to choose Maymester/Summer 2018 as the current semester. If the Disability Access Center notices incomplete information you may be contacted to fill out the form again. This ensures that the Disability Access Center will only contact the instructors that you want us to.
Upon completion of the form, the Disability Access Center will email you and your instructor your accommodation letters for each course you requested for. Then at the start of the semester, the Disability Access Center encourages you stop in and pick up printed copies of your accommodation letter so that you may give them to your instructors.
