Continuing Student Forms

Change of Address Form
Use this form to change one or more of your addresses in SEVIS – the Student and Exchange Visitor Information System of the United States government.

Program Extension Form
If you are unable to complete your degree by the date indicated on your F document (I-20) or J document (DS2019), you may apply for an extension.

Reduced Course Load Request Form
Reduced course load approval may be obtained by completing Sections 1 and 2 of this form and then meeting with a DSO.
