Zoom License Request

This form must be used for all Purdue University Zoom license requests.

  • All requests are subject to approval. All licenses will renew July 30 of each year automatically unless you notify CSC@PNW.EDU 30 days in advance.
  • Cost per license is $45/license per year. *
  • Please confirm using this chart that you really do require a paid Zoom license prior to requesting.
  • Each license you wish to purchase will need to be associated with a current Zoom user.  Each of those Zoom accounts must be registered to an @pnw.edu, @purdue.edu, or @pfw.edu e-mail address.
  • If a department or group will be requesting licenses, please appoint 1 individual to fill out this form on behalf of each user to limit the number of forms submitted.

* This cost may be covered centrally for Purdue University employees for FY20.  Contact your Business Office for more information.
