Workplace Injuries – New Process

Effective today, July 1, 2025, there will be a new process for communicating and following up on work related injuries.

Please review the information below and reach out to Human Resources with questions.


If an injury occurs while at work, please follow these instructions.

  1. If emergency medical treatment is necessary, call 911.
  2. If emergency medical treatment is not necessary, call the 24/7 Nurse Triage Line for on-the-job injuries (844) 454-1143.  It is recommended that the injured employee participate in this call.  Follow the instructions provided by the Nurse Triage Line.
    1. If medical treatment is necessary, the Nurse Triage Line will provide you with the authorized workers compensation provider locations that you may visit that are closest to your work or home.
  3. Once an injury is reported to the Nurse Triage Line, Human Resources will receive notification and will reach out to the employee’s supervisor if a First Report of Injury Form needs to be completed.
  4. If there were any witnesses to the injury, the supervisor should ask witnesses to complete the Witness Report Form and email the form to hr@pnw.edu or fax the form to (219) 989-2185.
  5. If the injury results in the employee being admitted to the hospital, please report the incident to the University Police.
