New Member Portal & Important Login Update
AllOne Health recently launched a redesigned member portal, making it easier to use and aligns with the AllOne Health App, making it simple to access counseling, mental health resources and well-being support.
All users will need to reset their password the first time they access the new member portal due to updated password requirements. Once the password is reset, members will use the same username and password for both the member portal and the AllOne Health App.

PNW’s Employee Assistance Program (EAP) through AllOne Health offers free short-term, confidential counseling through a network of licensed clinicians located close to your home or workplace. These trained professionals are ready to help you deal with family or work-life issues that may be causing your life to feel out of balance.

AllOne Health offers confidential care, and its clinicians have considerable expertise and clinical knowledge. The clinicians are well-versed in helping clients of all ages, from young children up to the elderly.

How it Works

On the home page of the AllOne Health portal, you can click the “Work Life Request” tile, located in the middle of the webpage. From there, you will be prompted to complete a short request form.

Via AllOne Health, the Employee Assistance Program (EAP) provider for Purdue Northwest campuses, individuals have access to a variety of assessments, including a depression screening survey. The survey is based on the PHQ-9 score, which is designed to provide a quick assessment of whether an individual might have signs and symptoms related to depression.
Additionally, the AllOne Health depression screening survey can be used to fulfill the Depression Screening challenge to earn incentives as part of the Your Path Wellness Program.*
Take the survey
To begin the depression screening survey, visit AllOne Health at https://portal.allonehealth.com/ and log in.
Once signed in, scroll down to the center of the page and click on the ‘Self Assessments’ tile. Click the ‘Depression’ tile and answer the nine questions. When finished, click ‘Submit’ then review your results.
To locate the results at any time, click on ‘Account’ at the top of the page, scroll down to ‘Completed Assessments’ on the lefthand side, and click on the assessment you wish to review.
Earn incentives
*To earn Your Path Wellness Program incentive funds from the AllOne Health Depression Screening, employees and covered spouses must record the screening completion in the Your Path Wellness Portal, under “Activities,” “Depression Screening.” This must be completed in the Your Path Portal to earn incentives. If you do not certify completion in the Portal, you will not earn the Depression Screening incentive funds.

- When you have tried various solutions, and none seem to work.

- You know something is wrong, but you can’t seem to identify the nature of the problem.
- You are having physical signs of stress including headaches, upset stomach, insomnia, etc.
- You need an objective point of view about a problem.
The AllOne Health EAP is completely confidential. When you contact AllOne Health, your name and details of your request will not be reported to your employer or anyone else in your family. No one will know unless you choose to tell them, or unless you give AllOne Health written permission to disclose certain information. Your employer offers this confidential service because they care about you and your concerns, but they don’t want to intrude into your personal issues.
Please note: AllOne Health and the clinical providers are required by law to report any cases of suspected child abuse, elder abuse, or threats of physical harm to one’s person or other individuals.
It’s important that you are comfortable with your EAP professional. We want this relationship to work for you. If you want to change to a new EAP professional, call us with your request. You will not be charged for that visit.
- There are 10 EAP sessions per issue per employee and 10 per eligible family member per contract year.
- The contract year runs from August 1 through July 31.
Once you have used your EAP sessions, you may choose to continue services under the terms of your health-plan benefit. You are responsible for fees incurred for additional sessions. AllOne Health makes every attempt to arrange your EAP sessions with a counselor who is in your health plan network so you may continue with the same person.
AllOne Health has a network of professionals often referred to as counselors or providers. These professionals include experienced licensed psychologists, social workers, counselors and therapists. When you call AllOne Health, we will match you with the provider who will best meet your needs. Appointments are scheduled at times convenient for you and at a location close to your home or workplace.
There is no cost to you or to your family members. No co-pays, fees, claims, or deductibles. The cost of this service is covered by your employer. If you need professional services that extend beyond the EAP, those services may be covered by your medical plan. If not, you will be responsible for costs of services outside of the EAP. It is important to note that you do not need to be a participant in your employer’s medical plan to use the EAP.

Those on a Purdue medical plan have resources through AmeriBen, the plan administrator for the Purdue University medical plan in conjunction with Anthem, Inc.
Behavioral Health Professionals in Your Plan Network
Through the AmeriBen website or AmeriBen Engage App on Google Play or in the App Store, you can review behavioral health professionals within your plan network by selecting “Find Care” and the search “Behavioral Health Visit.”
LiveHealth Online
LiveHealth Online offers live, instant or scheduled visits with doctors, licensed therapists, psychologists or psychiatrists from anywhere you have Internet access using two-way video conferencing. It is secure, private, easy-to-use and affordable.
- LiveHealth Online Psychology – provides online access to licensed therapists for common behavioral conditions such as grief, coping with illness, anxiety, stress, depression, relationship troubles and more.
- LiveHealth Online Psychiatry – provides online access to expert advice, treatment plans and medication if needed by board-certified psychiatrists for issues such as stress, depression, medication concerns, panic attacks and more.
Purdue University includeing Purdue Northwest cares about the overall well-being of its faculty and staff, and mental health is included. As a reminder, rest and taking time off can be very beneficial for individuals and their work.
Taking “mental health” days throughout the year when needed can help increase overall productivity and attendance at work. Individuals are accustomed to taking days off for physical illness, and that mindset should be the same for taking days off for mental health needs. Mental health parity and addiction equity – which all of Purdue’s health plans have – mandates that the health plan must treat mental health and addiction conditions the same as a medical condition. Purdue’s health plan coverage for mental health conditions and substance use disorders includes medication, office visits, testing, in-patient and outpatient treatment. One reason employees have sick time to utilize is so they can take time off and advocate for themselves and their mental health.

Suicide Prevention Resource Center (SPRC) – shares multiple resources on suicide prevention
The goal of the Strong Couples Project is to help strengthen the relationships of couples throughout the United States. Individuals 18 years of age or older who are married, engaged or who have been living with their partner for at least six months can participate. It is one of the most scientifically supported online programs for couple relationships and has helped thousands of couples to date. The program consists of six, self-paced online modules and five, brief video calls with a trained coach. Additionally, each couple will complete three brief surveys to examine the effectiveness of the program. All program activities are completed virtually. The program is led by the University of Illinois Urbana-Champaign and the University of Illinois Extension. Purdue Extension is a partner of the project. Learn more here.
