Required Forms
The Office of Financial Aid at Purdue University Northwest must request information or documents based on the verification type or documentation required by the Department of Education. Check Student Self-Service through the myPNW portal to see which documents or information are required to complete your financial aid file. Additional forms may be found at Forms and Instructions.
Instructions: Complete and turn in all required documents requested from you. Include your PUID number and name on all documentation submitted to the Financial Aid Office. All documents must be signed with a written signature.
2023-24 Verification
These forms are for students enrolled in fall 2023, spring 2024 and/or summer 2024.
Dependent Students Selected for Verification
Students required to report parent data on the FAFSA, are considered to be a dependent student for Federal Student Aid purposes.

Independent Students Selected for Verification
Students not required to report parent data on the FAFSA, that met at least one criteria to be independent, are considered an independent student for Federal Student Aid purposes.

2024-25 Verification
These forms are for students enrolled in fall 2024, spring 2025 and/or summer 2025. Once available the forms will be linked to the Banner Self-Service via the myPNW portal
